ABSTRACT

Objective To investigate the recurrence rate and recurrence risk factors of benign positional vertigo after manual reduction. Methods A total of 186 cases diagnosed with BPPV were selected as our subjects. Vertigo recurrence rate was calculated at 30 d,60 d,90 d after manual reduction. Risk factors for recurrence were analyzed using binary Logistic Regression. Results 172 cases diagnosed in BPPV patients completed 90 days follow-up. Vertigo recurrence rate was 8. 79%(16/182)in 30 days,17. 05%(30/176)in 60 days, and 22. 09%(38/172)in 90 days. The average age in recurrence patients was(59. 1 ± 10. 2)years old, significantly higher than the non-recurrent group( 47. 6 ± 7. 3 ). The recurrence rates in patients with complicated with hypertension, diabetes, hyperlipidemia, coronary heart disease were 57. 89%(22/38),60. 53%(23/38),73. 68%(28/38),39. 47%(15/38)respectively. Rise and turning neck are prone to recurrence, and recurrence rate were 16. 42%( 22/134 ),8. 21%( 11/134 ),9. 70%( 13/134 ) and 16. 42%(22/134). Recurrence rate of patients with manipulative reduction times ﹥10 times( reset multiple times before being cured)was 90. 90%( 10/11 ). Logistic regression analysis showed that the duration before manual reduction BPPV(OR=2. 631,95%CI 1. 082 -7. 572,P =0. 022)and the number of manual reduction ﹥10( OR=3. 107,95%CI 1. 205-7. 842,P=0. 018)after BPPV is manipulative reduction independent risk factor for recurrence. Conclusion Patients with BPPV in middle-aged and high seizure frequency have a high recurrence rate. Patients with BPPV complicated by hypertension, diabetes, hyperlipidemia, coronary atherosclerotic heart disease have the same higher recurrence rate. The rise and turn the neck often cause BPPV recurrence. The reduction and the number of manual reduction﹥10 are independent risk factors of recurrence of the BPPV.

ABSTRACT

OBJECTIVEThis study addresses the characters of magnetic resonance imaging (MRI) and clinical features, and curative effects in the screening of SSNHL due to inner-ear hemorrhage.

METHODSMRI and relevant audiometric test were given to 160 patients with SSNHL, who were hospitalized in the First Affiliated Hospital of Sun Yat-sen University from January 2011 to April 2013. The clinical features and curative effects of patients with high signals in the labyrinth on MRI were analyzed.

RESULTSMRI abnormities were found in 22 (13.8%) of the patients. Specifically, eight cases were considered for inner-ear hemorrhage. For the eight inner-ear hemorrhage cases, clinical features included the sudden onset of complete hearing loss, which worsened within several hours. Pure tone audiometry indicated profound sensorineural deafness. The prevalence of inner-ear hemorrhage was 5% in SSNHL cases and 18.6% in cases of profound sensorineural. MRI showed high signal intensity in the cochlear, semicircular canals or vestibule on unenhanced T1-weighted and fluid-attenuated inversion recovery images. The high signal intensity in the inner ear gradually degraded in six months. There was no improvement in hearing for the patients with inner-ear hemorrhage following symptomatic therapy.

CONCLUSIONSSNHL due to inner-ear hemorrhage is characterized by profound sensorineural deafness in all frequencies, and high signal intensity for the hemorrhagic inner ear on T1-weighted MRI, with poor prognosis.

ABSTRACT

Objective:To explore and test a caregiving experience model by Structural Equation Modeling (SEM)in the family caregivers caring for people with dementia in China. Method:Totally 349 elder patients with dementia and their family caregivers were selected and interviewed, using a battery of measures including demo-graphic data of the dyads, impairement of care recipient, caregiver's appraisal of role strain, role gain, negative and positive well-being outcomes. By using the Kramer's caregiver adaptation model as theoretical framework,a care-giving experience model were hypothesized tested by structural equation modeling (SEM). Result:A five-factor model for the caregiving experience was supported by SEM. The model demonstrated that the severity of dementia significantly influenced the caregiver's appraisal of role strain (β=0. 5 1 ,P<0. 05 ),which predicted indirectly neg-ative well-being outcomes of the caregiver (β=0. 63,P<0. 05). On the other hand the caregiver's appraisal of rolegain which was negatively influenced by the severity of parent's dementia (β=-0. 33,P<0. 05),predicted posi-tive well-being outcomes of the caregiver (β=0. 75 ,P<0. 05 ). Conclusion:It suggests that both positive and nega-tive aspects of caregiving experiences could independently impact on the well-being outcomes of adult-child care-givers,and therefore both aspects should be targeted in supportive interventions for family caregivers in China.

ABSTRACT

[Objective] To study the methylation status of CD40L gene regulatory regions in peripheral blood CD4+ T cells from patients with systemic sclerosis (SSc).[Methods] Peripheral blood mononuclear cells were isolated from the venous blood of 21 SSc patients and 20 healthy controls by density gradient centrifugation.CD4+ T cells were separated by using magnetic beads.Genomic DNA was isolated from the CD4+ T cells and treated with sodium bisulfite.Nested PCR was perfonned to amplify the desired regulatory sequences (including the promotor and enhancer) of CD40L,and the amplicons were transformed into the Escberichia coli DH5α.Subsequently,8 independent clones were selected and sequenced for each of the amplified fragments.[Results] In healthy female controls, half of the cloned fragments of CD40L regulatory sequences were unmethylated, and the other half were methylated. The mean methylation levels of CD40L promoter and enhancer from female SSc patients were significantly lower than those from healthy female controls (both P < 0.01 ).Almost all of the cloned fragments of CD40L promoter and enhancer were unmethylated in healthy male controls and male SSc patients, with no significant difference in the methylation level between male SSc patients and healthy controls (both P > 0.05,respectively).[Conclusion]s There is a low methylation level of CD40L regulatory elements on the inactive X chromosome in female SSc patients, which may contribute to the CD40L overexpression in CD4+ T cells.
